About the Program
To support you during this unique time in your life, Orlando Health South Lake Hospital Center for Women and Babies is offering a free program designed to provide follow-up care for the management of hypertension and other needs following your delivery.
Eligibility Requirements
To be considered for this program, you must live in Lake County and be pregnant or in the postpartum period. Postpartum is defined as up to one year after delivery.
How Our Program Works
Upon enrollment into the program, you will be given a blood pressure cuff to use at home. The program includes two free virtual visits or phone call visits within your first week of discharge. During the telehealth visits, the nurse will help with monitoring your progress and checking for potential complications. This will involve some personal questions about your circumstances in order to better help you throughout your postpartum journey, as well as the healing and coping experience after pregnancy.
The visits will cover the following:
- A review of your physical symptoms, including blood pressure, incision healing, medications and pain levels
- Answers to any breastfeeding or newborn care questions you may have
- A discussion of mental, emotional and psychological symptoms, such as mood swings or a history of anxiety, depression or eating disorders
- An assessment of nonmedical factors affecting your daily life, including food, housing, transportation and baby care items
